📌 Remember! If you are already a patient in any Gorebridge area surgery, you can't register with us. This applies to Newbattle or Pathhead surgeries' patients. 

2️ Step 2: Fill out the New Patient Questionnaire 📝

What's next?

Once the practice approves, you'll be officially a part of our list! We'll transfer your medical records to our practice, which can take a few weeks📁.  You may also get an invite for a new patient check-up/assessment🩺. 

Anybody in Scotland can access primary care services at a GP practice without charge.

When you register with a GP practice, it’s recommended that you provide information to confirm your:

  • identity

  • address

This means you can be matched to your existing NHS records. You shouldn’t be delayed or refused registration at a GP practice if you can’t provide identification or proof of address.

📎 Please bring two forms of identification.

 Acceptable ID includes:

- Passport 🛂

- Drivers Licence 🚗

- Official ID card from Public Services body 🏛️

- Student matriculation card (current year) 🎓

- Recent utility bill (within last 3 months) 🧾

- Council Tax document 🏡

- Television Licence 📺
